3DS Titles Disappeared From Home Menu

On the 3DS, Going to System Settings → Data Management → Nintendo 3DS → Software results in...

"The microSD/SD card software management information is corrupted. Reset it now?" message:

If you have any games or saves you'd like to keep, rebuild your Title Database. If you do not care about your current data, you can hit "Reset", which will erase the contents of your SD Card, rendering your current games and saves unplayable, and you can reinstall your apps afterwards.

"There is no available software data" message and/or installed custom theme is gone:

Your SD Card is not being read by your console. Ensure it is properly inserted into the console; if it is, backup the contents of your SD Card to a computer or phone, reformat the SD Card, then test its integrity.

Question marks or black boxes where game icons should be:
